Exam Site Registration allows both new and returning sites to confirm site details and designate personnel for exam administration, and is required for all participating exam sites. This process must be completed annually at least six weeks prior to testing. Check the BACE Calendar for other key Site Registration deadlines.

Annual Site Registration 

Exam sites must submit separate orders for each testing event. Check the BACE Calendar for other key ordering deadlines.

• First attempts: require six weeks’ notice
• Retakes: require two weeks’ notice

Please Note: You must register your site for the 2025 – 2026 academic year before placing an order, even if you were an exam site in previous years.

This form may be completed multiple times per academic year, specifically any time an Exam Site wishes to update information provided during Exam Site Registration.

Update Form

To modify an existing order, submit a Change Order Request. Be sure to have your order number available.

Timeline and Fees:

• Within 14 days of order placement: No fee (grace period)
• After 14 days: $175 change fee applies to all modifications
• Expedite requests (orders placed with less than six weeks’ lead time): $425 expedite fee
• Within two weeks of exam date: Requests cannot be honored

All requests are subject to availability as well as Biotility’s review and approval process.

To place a first attempt order after the six-week deadline has passed, submit an Expedited Order Request. Requests must be made at least two weeks before the testing event. A $425.00 fee will apply.

Expedite Order Request Form

Exam sites should submit an Accommodation Request for any candidate who requires testing accommodations. Requests are required at least two weeks in advance of testing date but should be submitted as early as possible to allow sufficient processing time. The accommodation form will guide you through the necessary details and documentation.
